As of January 2022 we are accepting applications for passenger vehicles and motorcycles. Personalized plates are available as well as sequential numerical plates.

Below you will find the most recent Specialty Plate Application Form from the Pennsylvania Department of Transportation (Penndot). The current form is dated July 2021 - the date can be located in the upper left corner of the form. Any previous forms cannot be used and will not be accepted. This is a two page form but you need only mail back the actual application (page 1).

The document labeled Plate Instructions will give you information on what is needed to accompany your application and the cost of the plate you are looking to get. Please note you must send back a photocopy of your current Blue Knights Membership Card (Current 2021 Cards are valid until April 2022 ) with the application and a check for the total amount due made payable to Blue Knight PA XIII Reference Specialty Plate Program. NOTE: Please keep your current registration up to date as the entire process can easily take up to 8 weeks!!
